There are 3 ways to order: (price $170 each includes UPS shipping) (there is no tax)

1. Call us at 928-536-2269 to order by credit card. Please be ready with all the information  requested on this form.

2. Print and fill out the following form. Mail it with credit card information, check or money order payable to:  Kerr-Cole Sustainable Living Center, P.O. Box 576, Taylor, AZ  85939

3. Send payment securely from credit card or checking account via electronic funds transfer. Print out this form first, as you will need to refer to it when ordering. Click Here for PayPal. Then Log-in, click on the Send Money tab and enter the requested information. Note the following:

Recipient Email: kerrcole@frontiernet.net

Enter Amount: $170  X  Quantity  =   $______________   Example, enter 340 for two units

Select Currency: U. S. Dollars

Select Category of Purchase: Goods (other)

E.mail Subject: Quantity ordered. Example: 2 Sport Cookers

Note: Put your Shipping Street Address in in the space provided for notes! Please provide street & number, city state and zip. PayPal is notably user-unfriendly in allowing you to enter this information in the ordering process. Put your Phone Number this box too; this is required for UPS delivery. Your E.mail Address would also be useful if we need to contact you regarding the order (Optional).

Ship to: If this box does not indicate your street address, click on Add Address and enter your Street Address. If this address has previously been entered, select it from the drop down menu. UPS will not deliver to a PO Box. Do not check the box referring to credit card billing address. Just skip to the Continue button. If PayPal cannot get your shipping address correctly, make very sure you have entered it in the notes space before you click on the Send Payment tab.

Whether or not we are set up to accept credit cards is questionable. PayPal will probably does not work because our accounts were switched to a new bank  There will be a price increase if the cooker is put back in production.
